| Opportunity | Country to Coast Queensland (CCQ) invites general practices in the Gympie, North Burnett, and Livingstone LGAs to apply for the co-design and trial of innovative Multidisciplinary Team (MDT) Primary Health Models. One general practice will be selected per LGA to partner in this initiative. |
| Purpose | This opportunity aims to strengthen chronic disease management, enhance patient care coordination, and build the capacity of the local primary healthcare workforce. The initiative responds to the complex needs of individuals living with chronic conditions, addressing physical and mental health, social support, transport, and financial challenges. |
| Funding and Term | General practices are required to enter into an Agreement for a two-year term. The following funding amounts have been allocated per general practice (one practice per LGA):

| Commissioning approach | Successful applicants will participate in a co-design session facilitated by CCQ to finalise the MDT model, agree on key performance indicators (KPIs), and establish reporting requirements. This commissioning method reflects CCQ’s commitment to system reform, community consultation, innovation, and flexibility while addressing local service gaps and promoting equity in service design and delivery. |

The Medicare UCC Program was first funded by the Australian Government in the October 2022–23 Budget. Since the initial announcement to establish 58 Medicare UCCs, the commitment has expanded to 137 UCCs nationally by 2025–26 with the March 2025 announcement of 50 new locations.
Medicare UCCs aim to improve access to urgent care in non-hospital settings by reducing pressure on Emergency Departments and providing patients with access to urgent care services at no cost. These clinics support integration with local health systems and complement general practice.
New locations in the CCQ region include Caloundra, Buderim and Gladstone.
Bulk billing preference: The Australian Government has directed that, where local circumstances allow, Medicare UCCs be co-located with or partnered alongside fully bulk billing general practices. Preference will be given to providers offering full bulk billing, with local bulk billing rates considered during commissioning to help ensure affordable primary care remains accessible.

Country to Coast Queensland (CCQ) has secured $200,000 through the PHN Multicultural Access Program (MAP) for 2025–26 to improve health and wellbeing for culturally and linguistically diverse (CALD) communities and has dedicated the funding to focus on the health and wellbeing of PALM scheme workers in Bundaberg.
CCQ invites proposals from qualified providers to:
- Identify and address health and wellbeing needs of PALM Scheme workers
- Improve access to primary healthcare for PALM Scheme workers
- Strengthen cross-sector collaboration and service integration
- Enhance the disaster and climate resilience of PALM Scheme workers to protect health
Providers should have strong capability in project delivery, co-design with CALD groups, and leading multi-sector health initiatives.
